As a nurse it is your responsibility to ensure that clients know/understand their rights and to honor those rights while implementing care. They include the rights to respectful, considerate, competent care, to participate fully in the decision-making process, to accept, refuse, or request modification of the plan of care. The nurse introduced himself, performed hand hygiene, and followed the facilities protocol for identifying the client.
Nurse: Mrs. Lindermen how are you feeling today
Patient: The pain is unbearable, I just cant do this anymore
Nurse: would you like me to give you some pain medication
Patient: Yes please, but no more chemotherapy, its just too much for me
Nurse: Well I spoke to Dr Klein and he told me how you feel
Patient Daughter: Can I speak with you outside?
